The rates for the primary insured and spouse are based on the attained age (last birthday) of each
individual. Premiums will change on December 15 after an insured person moves into a higher age group.
All insured dependent children (up to age 21; 27 if a full-time student) are covered by one economical premium amount –
$24.50 for all!
NOTE: A $3.00 administrative fee will be added to each premium notice.
